Application Requirements

• 4 passport-sized colored photos (4.5cm x 3.5cm; white background; no glasses, scarfs or hats allowed; both ears visible)
• Colored copy of the old passport (visa and bio-data page + page 45)
• Colored copy of a valid ID issued in Korea (e.g. residence card or alien card)
• Filled Form G (download in the download section of this website)
• Duly fill out the passport renewal application form
• Colored copy of a valid ID issued in Korea (e.g. residence card or alien card)
• Return envelope with postage for registered mail

The fee is ¥5,000, and it can be done in person, by post, or by email.

Application Requirements

• Colored copy of a valid ID issued in Uganda (e.g. National ID)
• Colored copy of a valid ID issued in Japan (e.g., residence card)
• Duly filled out Form G
• Letter explaining the circumstances of the lost passport
• Police report on the lost passport
• Return envelope with postage for registered mail

The fee is ¥5,000, and it can be done in person, by post, or by email.

• Birth Certificate of the child, showing the mother and the father
• Translation of the Birth Certificate to English if in another language
• Colored copies of the passports of the father and mother (if Ugandan passport, the visa and bio-data page and page 45)
• Colored copies of a valid ID issued in Korea (e.g., residence card) of the father and mothere
• Return the envelope with postage for registered mail.
